What is the difference between Form N-565 and Form N-600 and their purpose:

To obtain a Certificate of Citizenship, USCIS provides Form N-565- or N-600. The purposes of each forms are different which we describe briefly below.

Form N-565: Application for Replacement Naturalization/ Citizenship Document. This form is used to apply for replacement of US citizenship documents such as a Certificate of Naturalization or a Certificate of Citizenship.

Purpose: A US citizen can file form N-565 if the Certificate of Naturalization or a Certificate of Citizenship was lost, stolen, destroyed or mutilated; to correct any typographical/clerical error by USCIS; or for name change due to marriage or by court order. However, USCIS cannot make any changes to correct date of birth on these documents as an individual is obliged to provide truthful and correct information at the time of filling N-400 and also by signing at the time of interview.  Further, when applying for N-565, you are required to support the application with the damaged documents, court order or any other supporting documents based on which a request is being made to USCIS.

Filing fee: The filing fee for Form N-565 is $555, payable to US Department of Homeland Security as a check or a money order.
