Our office has received an update that some consulates are now requesting a Supplemental Questions for Visa Applicants form to be completed by applicants at the time of stamping. This form, copied below, is extensive, requesting for information through 15 years back. This extensive list of questions is a cause for concern, and in particular the following questions:
“Have you travelled to any country (other than you country of residence) in the last 15 years? If yes, provide details for each trip, including locations visited, date visited, source of funds, and length of stay.”
“Siblings (Brothers and Sisters) – Provide the full name(s) and date of birth of any sibling (full, half, step, adopted), living or deceased.
Date of Birth (mm-dd-yyyy):”
“Address – Provide all addresses where you have lived during the last 15 years, if not already provided in your application.
Dates of Residence (mm-dd-yyyy)
City State/ Province:
Postal Zone/ Zip Code:
“Social Media – Please provide your unique user name for any websites or applications you have used to create or share content (photos, videos, status updates, etc.) as a part of a public profile within the last five years.”
At this time we do suggest that before planning your out of country trip, please speak to your immigration attorney.
